You can fill up and pay online

STEP BY STEP KAISER ONLINE APPLICATION:

STEP 1. Mag-login sa Online Application ng Kaiser at ilagay ang sumusunod na impormasyon, para makapag-create ng personal proposal plan.

STEP 2. After niyan ay may lalabas ulit na tab kung saan mag-fill out ka ulit ng detail katulad ng Name, Email, Birthdate, Plan Name, Payment Form.

Note: Pwede mong palitan ang PLAN NAME, iclick lang ang drop down menu (Plan 100,000 kasi ang default)

STEP 3. Kapag na-set na ang lahat ay i-click ang COMPUTE QUOTE.

STEP 4. Once na-click mo ang compute quote ay mag-uupdate ang mga benefits ng Plan sa right side. Kung hindi ka satisfied sa pinakitang benefits ay maaari ka ulit pumili ng PLan, click mo lang ulit ang Plan Name and choose your desired plan. Again, click Compute Quote.

STEP 5. Kapag naka-set na ang lahat ay ibrowse ang lower portion may makikitang kang terms of payment like Monthly, Quarterly, Semi-annual, Annual and Spotcash.

Note: May discounts kapag Quarterly, Semi-annual, Annual and Spotcash. Sa annual ay 2 months ang discount at syempre mas malaki ang discount ng spotcash.

STEP 6. Once na nakapag-decide ka na kung anong Terms of Payment, just click CHOOSE.

STEP 7. Pag-click mo ng choose button may mag-appear na "An invitation to complete the kaiser application of <your name> has been sent to: <your email>". Ibig sabihin ay ready to fill out na ang iyong Kaiser Forms. Just click the COMPLETE CLIENT APPLICATION NOW button or you can check your email to Complete your Kaiser Application Form.

STEP. 8 Just fill out all the required information and proceed to the payment.
